Use this worksheet to agree the responsibilities around one WordPress website. Copy the records into an approved project document or download the editable Markdown version. The page does not collect or save your answers on this website.

## Describe the website and responsibilities

- **Website name and public address:**
- **Business purpose and important visitor tasks:**
- **Business decision owner:**
- **WordPress maintenance owner:**
- **Hosting and domain owners:**
- **Other service owners:**
- **Review date and next review:**
- **Known gaps or assumptions to confirm:**

Use the [WordPress maintenance plan guide](https://blitzstudio.xyz/blog/what-to-include-in-a-wordpress-maintenance-plan) to connect these records to the site's actual needs. Keep account secrets and real customer information out of this document.

## Record the maintained components

Copy this record for WordPress core, the active theme, important plugins and connected services where relevant.

- **Component and purpose:**
- **Current version or configuration record location:**
- **Account or service owner:**
- **Where release information is reviewed:**
- **How updates are applied in this setup:**
- **Who receives update-attempt or failure notifications:**
- **Who reviews the result:**
- **Relevant test tasks:**
- **Renewal or support dependency:**
- **Open issue and decision owner:**

Confirm the actual configuration. Do not mark a component maintained merely because an automatic-update option exists or a hosting package uses a reassuring name.

## Define a recoverable backup set

- **WordPress files included:**
- **Database included:**
- **Other service data handled separately:**
- **Backup frequency and its business reason:**
- **Retention and approved storage locations:**
- **Who can access and restore the set:**
- **Who checks that expected backups exist:**
- **How failures are detected and assigned:**
- **Latest completed recovery rehearsal and evidence:**
- **Unresolved recovery dependency:**

The official [WordPress backup guidance](https://developer.wordpress.org/advanced-administration/security/backup/) explains why typical WordPress recovery involves files and a database. Record what was actually checked. A backup job result and a demonstrated recovery are different pieces of evidence.

## Specify the visitor tasks to review

Copy this record for each important journey.

- **Task and reason it matters:**
- **Starting URL and expected path:**
- **Approved synthetic test information:**
- **Relevant browser, viewport or input method:**
- **Expected outcome:**
- **Actual outcome and evidence:**
- **Who confirms downstream delivery where relevant:**
- **Issue owner and follow-up date:**

Examples include finding a service on a phone, following its contact route and testing an authorized enquiry through the agreed delivery process. Add tasks that reflect the actual site. Do not claim that a purchasing or account flow passed because its opening page loaded.

## Agree the change and incident record

- **Change or incident description:**
- **Component or service affected:**
- **Business impact reported:**
- **Who approved the planned work or response:**
- **Backup or recovery evidence checked before change:**
- **Time and person performing the work:**
- **Tasks reviewed afterward:**
- **Observed result:**
- **Escalation or recovery decision and owner:**
- **Unresolved issue, owner and next review:**

The recovery decision should account for current information and any new data created after a backup. This record does not authorize an operator to restore, delete or change a live site without the project's actual approval process.

## Fictional worked review item

This sample concerns a fictional consultancy with an enquiry form. Replace all sample decisions with the reviewed requirements of the actual business.

- **Item:** Check the enquiry journey after a relevant form-component change.
- **Reason:** The consultancy uses that route to receive project requests.
- **Task:** Open the service page at a narrow phone width and follow its contact action.
- **Test information:** Use the synthetic enquiry approved by the delivery owner.
- **Expected evidence:** A recorded interface result and separate confirmation from the agreed submission destination.
- **Responsibility:** The maintainer records the test; the receiving team confirms the downstream result.
- **Open issue:** The team must agree how test records are labeled and removed.

No test result is claimed by this fictional example. A browser success message alone does not prove receipt by the team.

## Review dependencies outside WordPress

- **Domain and hosting renewal ownership:**
- **Certificate and availability alert ownership:**
- **Notification routing and monitoring owner:**
- **Important third-party integrations:**
- **Where authorized access is managed:**
- **Business contact and service-copy review owner:**
- **Important links and public sitemap review owner:**
- **How a change in responsible personnel is handled:**

Keep the evidence location and responsible people current. Use the [website launch checklist](https://blitzstudio.xyz/resources/website-launch-checklist) when a maintenance change becomes a wider release. Use the [form planning worksheet](https://blitzstudio.xyz/resources/website-form-planning-worksheet) if the enquiry workflow itself needs clarification.
